Who Can Apply?

Now Available in Calgary and Across Alberta

  • Be a resident of Alberta with a valid Alberta Health Care card
  • Qualify for home care services through an AHS (Alberta Health Services) evaluation
  • Complete a home care assessment that recommends the CDHCI Program
  • Have no conflicts with AHS policies or service agreements
  • Have consistent, ongoing care needs
  • You’ve had a home care assessment done by Alberta Health Services (AHS)
  • We’ll help you understand your options and walk you through the steps.

How to Get Started with CDHCI

  • 1. Contact AHS

    Reach out to your AHS case manager or nurse care manager to see if you’re eligible. You can also call the AHS Community Home Care Intake line to self-refer.

  • 2. Home Visit & Assessment

    An AHS case manager will come to your home to assess your needs and let you know how many hours of care per week or per month you qualify for.

  • 3. Sign the Paperwork

    You’ll be asked to sign important forms, including:
    • A Letter of Agreement (outlining the CDHCI Program details)
    • A Consent to Disclose Information form
  • 4. Choose a Care Provider

    You’ll get a list of approved care providers from Alberta Blue Cross. (Yes—we’re on the approved list!) We strongly encourage you to choose Canadian Home Caregivers. With our compassionate team, personalized approach, and 24/7 support, we’re committed to making care easy, safe, and stress-free for your family.

  • 5. Make a Care Plan

    Work with your chosen care provider (in this case Canadian Home Caregivers) to create a schedule and care plan. A backup plan is also required in case services are interrupted.

  • 6. Pay for Extra Services

    If your care provider offers services not covered by the CDHCI Program (like overtime or medical equipment), you’ll need to cover those costs yourself.

  • 7. Keep in Touch

    Let AHS and Alberta Blue Cross know if there are any changes to your health, address, or care provider.
